Jerry Dwornik, 55, died Oct. 16, 2013.
Survivors include his siblings, Marilee and Rick Greene, Pittsville, Phil and Deb Dwornik, Eastman, Janel and Steve Grunewald, East Troy, Becky and Jerry Morris, Elkhorn; several nieces and nephews; and his stepfather, Dick Beller.
He was preceded in death by his mother, Janet Beller.
Graveside services will be at St. Peters Cemetery at 10 a.m. on Monday, Oct. 28.
